The thing I love most about Valve's trailers (especially for the L4D games) is how they do such a good job of quickly and subtly introducing you to all of the characters (survivors and infected), settings, challenges, and all of the gameplay mechanics. Obviously, that's the job of all trailers, but it's rare enough to see it done well for a movie, much less a game that requires a set of rules and mechanics to be conveyed as well!
I hadn't even realized how effective the first one was at all of this until I saw the many steps and revisions they went through to make sure all of the information was conveyed. But as always, Valve is one of the best in the business, and it shows not just in the grand, obvious ways, but in the subtle polish of their final work.
It really makes me sad I don't have the reflexes or the time to spend generating the reflexes for video games, because that right there is pretty much right freakin' up my alley.

Which means they're rendering it with the Source engine, just using much higher resolution assets and a better lighting model than can be used in real time.

There are some great little character touches in the animation there, a lot more than were in that recent Resident Evil straight-to-DVD CG thing. Very impressive.
Shame I'm in Australia, where playing this will apparently corrupt my 38-year-old soul, so it's been banned for my protection. I guess you'll all have to play it for me.
At GDC 08 or 09, some of the folks from Epic talked about using the engine to make promotional videos for Gears Of War. In particular, they mentioned that the doing so amazed their marketing partners at how fast and cheap it was. So my guess is way below what you'd expect if they did it in engine. Unfortunately, the old slideshows appear to have been buried on the site or deleted altogether, so I can't provide specific ball park numbers.
Valve games in particular and fpg games in general are for computers not x-box. There's no need to buy an Alienware super-expensive machine. a sub $1000 machine will run L4D2 like a dream.
Regarding cost per minute, the majority of the cost is getting the pacing and cinematography right. The rendering is all done in game. They might use super high res models and textures that run at less than full speed but it isn't like a Pixar movie that takes hours to render a single frame.
